Overweight, Metabolic Syndrome and Insulin Resistance

Overweight, metabolic syndrome, and insulin resistance are three different expressions of the same underlying process.

It is not simply a matter of “gaining weight.” Hormonal, metabolic, and even emotional factors may be involved in this situation.

We now know that abdominal fat is not just an energy reserve. It also produces inflammatory hormonal and protein signals that can worsen many chronic conditions.

For decades, restrictive low-calorie diets have dominated medical and nutritional advice. These approaches often lead to constant hunger and are difficult to maintain over time. At the same time, we have been told to follow the mantra of “eat less and move more,” together with the common habit of counting calories and trying to burn them through exercise.

However, the human body does not function like a steam engine that simply burns the calories we consume. Eating fat does not automatically mean that fat will accumulate in the body. We are complex metabolic systems that respond differently to different macronutrients.

For example, a calorie from sugar is not metabolically the same as a calorie from olive oil. Each follows very different metabolic pathways.

By carefully studying laboratory tests to evaluate metabolic status and possible underlying conditions (such as subclinical hypothyroidism), we can design a personalized lifestyle strategy for each patient.

A sustainable approach — without hunger and without the need to count calories.

The key to success does not lie solely in a metabolic assessment and a set of dietary and lifestyle recommendations.

It also requires continuous support from the physician—to address questions and challenges that may arise, to correct aspects that may not be progressing as expected, and to ensure that patients feel the reassurance of professional guidance, especially during moments of difficulty.

Learning how to eat well is not always easy. In many cases, it involves overcoming habits or dependencies that were previously unrecognized, such as cravings for sugar, bread, fruit, or alcohol.

Combining nutritional strategies with ongoing medical and psychological support, and incorporating intermittent and prolonged fasting once metabolic flexibility has been achieved, is one of the most direct and safe ways to reach an excellent metabolic state, regardless of age or body weight.

Fasting strategies lasting 48–72 hours, introduced after several weeks of metabolic preparation, can provide a reasonable and effective way to support weight loss. When properly supervised, this process helps reduce body fat while allowing the skin to adapt gradually, promoting a more balanced and natural body composition as weight is lost.
